Cost:

Ages 3-17: $380

Ages 18+: $400

Immunity testing (if needed): TBD

Form Replacement: If an applicant requests a replacement of sealed and completed USCIS forms for any reason other than an error made by our office, a $100 reprocessing fee will apply.

Vaccinations:

Vaccines are not administered at our facility. If you need vaccinations, please check with your health insurance provider to find out where you can receive them. Many of our patients choose to get their vaccines at their primary care clinic or a local pharmacy. If you don’t have health insurance, we can provide information on locations that offer vaccines at no cost or at a reduced rate.

Form Completion Timeline:

We will complete your USCIS form within 7 days after your appointment, provided all lab results are normal and required vaccines are received within 6 days. If vaccines are administered more than 7 days after your visit, that is perfectly fine. Please send us the records when available. In that case, your completed forms will be ready within 2 business days of receiving the documentation.
